Our agency is looking for a registered nurse. Â The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Â The professional nurse builds from the resources of the community to plan and idrect services to meet the needs of individuals and families witin their homes and communities.
The Registered nurse mjst be able to complete an inital assessment of patient and family to determine home care needs. Developes a care plan.
Registered Nurse must provide a current nursing license.

This person must be able to assess and evaluate therapeutic / rehabilitative / function status, and participate in the development of the total plan of care.
This person must possess a degree from a baccalaureate of masters program in physical therapy. Â Tow years of appropriate experience as a physical therapist. Â Community / home health experience is preferred. Â Demonstrates good verbal and written communication and organization skills. Â Posses and maintains current CPR certification.
